Suddenly everything looks bigger, possibly uneven, maybe a little bruised. This is completely normal, and understanding exactly what to expect day by day makes the difference between a calm recovery and unnecessary panic.

Why Do Lips Swell After Filler?

Hyaluronic acid is hydrophilic — it attracts and binds water. When injected, the surrounding tissue responds with localised oedema (fluid accumulation) as part of the normal healing process. The injection itself also creates micro-trauma, triggering the same inflammatory cascade your body uses to repair any minor injury.

This is not a sign that anything went wrong. Swelling is temporary and predictable.

Day-by-Day Recovery Timeline

Day 1 (Immediately After)

Swelling begins within minutes of treatment and will continue building for the first 12–24 hours. Your lips will look significantly fuller than your intended result — sometimes dramatically so. This is not what your final result looks like. Bruising may appear at injection points.

What to do: Apply an ice pack (wrapped in a cloth — never directly on skin) for 10 minutes on, 10 off. Keep your head elevated if resting. Avoid touching or pressing on your lips. Drink plenty of water.

Days 2–3 (Peak Swelling)

This is typically the worst it gets. Swelling peaks at 24–48 hours. You may notice asymmetry — one side swelling more than the other — and the lips may feel tight or tender. This is normal and resolves as the filler integrates and swelling decreases.

What to do: Continue ice for the first 48 hours if comfortable. Sleep on your back with your head slightly elevated. Eat soft, room-temperature foods. Avoid anything very salty (salt retains water and can prolong swelling).

Days 4–7 (Significant Reduction)

Swelling reduces noticeably. The lips start looking more like your intended result. Minor lumps or firmness in the filler material is common at this stage — this is the HA settling and will continue to soften.

Bruising, if present, begins to fade. Any yellowish discolouration is a normal stage of bruise resolution.

Days 8–14 (Settling)

What to Avoid After Lip Filler

First 24–48 Hours

  • Alcohol — dilates blood vessels, increases bruising and swelling
  • Intense exercise — raises blood pressure and body temperature, worsening oedema
  • Saunas, steam rooms, hot baths — heat increases swelling
  • Very hot or spicy foods — same reason as heat exposure
  • Kissing or any pressure on the lips — can displace filler before it sets
  • Ibuprofen/aspirin (unless medically required) — blood thinners worsen bruising

First Week

  • Dental procedures — pressure and stretching of the lip area can affect filler positioning
  • Lip waxing or threading — avoid until fully healed
  • Prolonged sun exposure — use SPF if you must go outdoors
  • Lip massage — unless specifically instructed by your practitioner

What You CAN Do

  • Light walking from day 2
  • Normal makeup from 48 hours (light touch around the lip area)
  • Resume normal skincare from day 3 (avoiding direct lip application)
  • Drink through a straw if eating is uncomfortable in days 1–2

Managing Bruising

Not everyone bruises, but it's common. Arnica gel or cream (applied around — not on — the injection sites from day 2) can help. Avoid heat during the first 48 hours. Covering with colour-correcting concealer is safe from 48 hours.

Bruising typically resolves in 5–7 days. If bruising is spreading significantly or the area feels warm and hard, contact your clinic.

When to Contact Your Clinic

  • White or grey patches on the skin (potential vascular occlusion — rare but requires immediate attention)
