Lip swelling after filler is normal — it's expected physiology. But how you care for your lips in the days after treatment directly affects both the quality of the result and how quickly you recover. This guide covers lip filler aftercare based on the protocols used at Cosmo Clinic Lisbon.

Lip filler uses hyaluronic acid (HA) — a substance that occurs naturally in the body. Unlike botulinum toxin, HA gel does not migrate due to increased blood flow. The main risks in the first hours are swelling, inflammation, and bruising. The aftercare rules below are built around minimising these.

What NOT to do after lip filler

Don't press or massage your lips for 24–48 hours

Touching, squeezing or massaging the lips can displace the hyaluronic acid gel before it integrates into the tissue. Avoid unnecessary contact with your lips — and avoid kissing with pressure for the first 24 hours. Don't smoke immediately after treatment (the suction creates pressure).

No lipstick or lip makeup for 4–6 hours

Injection sites are micro-wounds. Applying cosmetic products directly over them in the hours immediately after treatment increases the risk of irritation or infection. Wait at least 4–6 hours. After that, you can apply lip products normally — use a gentle touch when applying.

Avoid alcohol for 24 hours

Alcohol is a vasodilator — it increases blood flow and the likelihood of bruising. For the first 24 hours after lip filler, skip wine, beer or spirits. Moderate consumption can resume the following day.

No intense exercise for 24–48 hours

Exercise increases blood flow and vascular pressure, which worsens swelling and bruising. Light walking is fine. Avoid running, HIIT, contact sports, or any high-intensity training for 48 hours after treatment.

  • Use a cold compress (wrapped in cloth) if swelling is uncomfortable: for the first two days, gentle cold can reduce swelling. Never apply ice directly to the skin. 10-minute sessions with breaks are fine.
  • Sleep with your head slightly elevated for the first few nights: an extra pillow reduces the swelling that accumulates overnight.
  • Keep lips moisturised with a plain lip balm: a fragrance-free balm can be used from 4–6 hours after treatment to prevent dryness and support comfort.
  • Avoid sleeping face-down for the first few nights: pressure from the pillow against your lips can push on freshly injected tissue. Sleeping on your back is ideal for the first 1–2 nights.

Day-by-day recovery: what to expect

Lip filler recovery follows a predictable pattern. Knowing what's normal at each stage helps you tell normal healing from something worth checking:
