Skinboosters and biostimulators are not interchangeable. One category saturates the dermis with hydration and improves skin quality. The other triggers the body's own collagen machinery. Understanding which does what — and why a physician chooses one over the other — is the difference between a protocol that works and one that doesn't.

What are skinboosters and biostimulators

These are two distinct categories of injectable treatment, often grouped together because both are used for skin rejuvenation — but their mechanisms, timelines and clinical objectives are fundamentally different.

Skinboosters are injectable products based on non-cross-linked or minimally cross-linked hyaluronic acid. They do not add structural volume. They act superficially in the dermis, dramatically increasing local hydration, improving skin elasticity and restoring the skin's natural luminosity. The most widely used at Cosmo Clinic are Profhilo (IBSA) and NCTF 135HA (Filorga).

Biostimulators are a distinct class: they contain no HA. Instead, they deposit a biocompatible scaffold in the deep dermis or subcutaneous tissue that stimulates the body's own fibroblasts to produce new collagen and elastin. Results appear gradually over weeks to months — and last significantly longer. The main products approved and used in Portugal are: Sculptra (PLLA — poly-L-lactic acid), Radiesse (CaHA — calcium hydroxylapatite), Ellansé (PCL — polycaprolactone) and HarmonyCa (CaHA + HA hybrid).

Skinboosters improve what is already there. Biostimulators rebuild what has been lost. The choice between them is not a preference — it is a clinical assessment.

How they work in the tissue

The two categories work through entirely different biological pathways. Understanding both helps patients set realistic expectations — particularly around results timelines.

Skinbooster mechanism (Profhilo / NCTF):

  • Profhilo contains one of the highest concentrations of HA in any injectable product (64 mg/2 ml) — a hybrid of low and high molecular weight HA chains. It is not a filler: it disperses through the tissue rather than remaining localised. It acts on four skin cell types simultaneously (fibroblasts, keratinocytes, adipocytes and pre-adipocytes), stimulating collagen I, III and IV, elastin and adiponectin production.   • NCTF 135HA is a polynucleotide and micronutrient cocktail: 55 active ingredients including vitamins (B1, B2, B3, B5, B6, B8, B9, B12, C, E), amino acids, minerals, coenzymes, nucleic acids and low-molecular-weight HA. It is injected via multiple micro-punctures across the entire treatment area. It works by supporting fibroblast metabolism and improving cellular environment — resulting in brighter, more even skin with refined pores and improved texture.

Biostimulator mechanism (Sculptra / Radiesse / Ellansé / HarmonyCa):

  • Sculptra (PLLA) works through a foreign body response: the poly-L-lactic acid microspheres are gradually resorbed over months, and during this process the body surrounds them with a collagen matrix. The net result is a progressive increase in dermal and subdermal thickness. Full results appear at 3-6 months and can last over 2 years.
  • Radiesse (CaHA) provides an immediate volumising effect (calcium hydroxylapatite gel carrier) combined with long-term collagen stimulation as the CaHA microspheres are resorbed. Duration 12-18 months. Also used as a "diluted Radiesse" skinbooster technique for neck and hand rejuvenation.
  • Ellansé (PCL) uses polycaprolactone microspheres in a CMC gel carrier. Unique among biostimulators for its predictable, controlled duration (S: 1-2 years, M: 2-3 years, L: 3-4 years). Excellent for mid-face volume and jawline definition.
  • HarmonyCa combines CaHA with HA in a single product — provides immediate structure plus biostimulation in one injection. Used for cheeks and mid-face.

The key clinical distinction: skinboosters work primarily on skin quality (hydration, radiance, texture, fine lines). Biostimulators work on tissue volume and structural elasticity. A 35-year-old with dull skin needs the former. A 50-year-old with significant volume loss needs the latter. Many patients benefit from both, in sequence.

  • Cheeks and mid-face — primary target for biostimulators: Sculptra, Radiesse and Ellansé for volume restoration and structural support. HarmonyCa for combined structure + stimulation in one session.
  • Temporal zone — Sculptra is well-suited to temporal hollowing, gradually rebuilding the convexity lost with age without the risk of vascular complications associated with HA fillers in this zone.
  • Periorbital area — NCTF around the eyes for fine lines and crepey texture. Not the same indication as HA tear trough filler.
  • Jawline and lower face — Ellansé or diluted Radiesse for structural definition with long-duration biostimulation.

Neck and décolletage:

  • Profhilo and NCTF are particularly effective on the neck — an area that ages visibly but is poorly served by most other treatments. The skin of the neck is thinner and has fewer sebaceous glands than the face; skinboosters improve its quality without adding inappropriate volume.
  • Diluted Radiesse (hyperdilute technique) stimulates collagen and tightens loose skin on the neck and décolletage. Requires 2 sessions in the initial protocol.

Hands:

  • The dorsum of the hands loses volume and skin quality with age, making tendons and veins prominent. Profhilo and NCTF restore skin quality. Radiesse restores volume and improves skin texture simultaneously.

Pre-treatment — what to do before

  • Avoid anticoagulants (warfarin, heparin, preventive aspirin) and NSAIDs (ibuprofen) for 5 days before treatment — these increase bruising risk. Always confirm with your prescribing doctor before stopping any medication.
  • No needle facial procedures (mesotherapy, deep peels, laser) in the 2 weeks before the session.
  • Good skin hydration in the days before — improves tissue quality and product integration, particularly for Profhilo.
  • Inform the clinic of any active autoimmune condition, known allergies to product components, or if you are on immunosuppressant therapy.
  • For biostimulators (Sculptra in particular): avoid vigorous facial massage in the 2 weeks prior — it can affect tissue planes at the injection sites.
  • In-person assessment consultation mandatory — we never administer biostimulators without prior clinical evaluation.

During the session

What happens at the appointment

  • Duration: 30-45 minutes depending on the product, zones treated and number of points.
  • Topical anaesthesia applied 15-20 minutes in advance — included in the session price.
  • Cleaning and antiseptic preparation of the treatment area.
  • Profhilo: application at 5 BAP points per side using a fine needle. Gentle pressure post-injection to encourage product distribution.
  • NCTF: micro-puncture technique across the treatment zone with a thin mesotherapy needle.
  • Biostimulators: deeper injection with cannula or needle depending on the product and zone. Massage required after Sculptra to distribute product evenly.
  • Post-session clinical photograph for protocol tracking.

Recovery: what happens day by day

Recovery from skinbooster and biostimulator treatments is generally mild compared to structural fillers. The expected post-treatment response differs slightly by product.

Post-treatment — protocol by period

First 24 hours (critical phase)

  • Small papules at injection sites (particularly Profhilo BAP points): expected, resolve within 24-48 hours. Do not touch, press or massage the treatment area.
  • Mild redness and swelling at the puncture sites: normal. Apply cool compress (wrapped in cloth, not direct ice contact) if uncomfortable.
  • No makeup on the treated area for 12 hours — needle punctures temporarily open channels that increase infection risk.
  • No intense physical exercise — increased blood pressure to the face prolongs redness and swelling.

First 48-72 hours

  • No sauna, steam room or prolonged hot bath — heat increases vasodilation and swelling.
  • No alcohol — worsens bruising and delays resolution of any swelling.
  • No direct sun exposure or sunbed — the treated skin is temporarily more reactive.
  • Sculptra specific: massage the treated area for 5 minutes, 5 times daily, for 5 days after the session (the "5-5-5 rule") — essential to ensure uniform product distribution and prevent nodule formation.

First 2 weeks

  • No deep facial massage or ultrasound therapy over the treated zone.
  • No ablative laser or chemical peel in the same area.
  • Sculptra: avoid vigorous facial movements and deep tissue massage for the full 2 weeks.

Results timeline by product:

  • Profhilo — improved radiance and skin hydration visible within 2-3 weeks of first session. After session 2 (at 1 month): measurable improvement in elasticity and skin quality. Full bioremodelling effect develops over 3 months.
  • NCTF — skin brightness and texture improvement visible from 2 weeks. After the 4-session protocol: significant refinement in pore size, pigmentation and overall skin quality.
  • Sculptra — minimal immediate effect. Collagen stimulation begins at 4-6 weeks, with progressive volume and structural improvement continuing for 3-6 months after the final session. This is the intended mechanism — gradual, natural-looking improvement.
  • Radiesse — immediate volumising effect (gel carrier), plus progressive collagen stimulation visible from 6-8 weeks. Full biostimulation develops over 3-4 months.

Who it's for — and who it isn't

Not every patient is a candidate for every product. Clinical assessment determines the appropriate protocol — there is no single "skin booster for everyone."

Main indications for skinboosters (Profhilo / NCTF):
