Lip Flip vs Lip Filler: Which Is Right for You?
Mechanism, results, duration, price and ideal candidates — a clinical guide with comparison table.
Lip flip and lip filler are frequently confused — but they are treatments with completely different mechanisms, results, and durations. Choosing between them (or combining both) depends on what each person wants to achieve. This article explains the differences clearly.
How each treatment works
Lip flip
A lip flip uses small doses of botulinum toxin injected into the orbicularis oris muscle — the circular muscle that surrounds the mouth. Partial relaxation of this muscle causes the border of the upper lip to curl slightly outward (eversion), creating the appearance of a more projected and fuller lip. No real volume is added. The effect is subtle and natural.
Lip filler
Lip filler uses hyaluronic acid injected directly into the lip tissue to add volume, define the contour, and increase projection. The doctor shapes the product to achieve the desired result — more volume, better vermillion border definition, or both. It is a highly customisable treatment.
Direct comparison
| Lip flip | Lip filler | |
|---|---|---|
| Product | Botulinum toxin | Hyaluronic acid |
| Effect | Upper lip eversion | Volume + definition |
| Adds real volume? | No | Yes |
| Duration | 2–4 months | 6–12 months |
| Recovery | Minimal (1–2 days) | 3–7 days (swelling) |
| Reversible? | Yes (effect wears off) | Yes (hyaluronidase) |
| Relative cost | Lower | Higher |
Who is each treatment best for
Lip flip — best for:
- People who want a very subtle result without adding visible volume
- People who already have good lip volume but want more natural projection of the upper lip
- People getting botox anyway who want to include a lip treatment in the same session
- People who want to try a result on their lips before committing to filler
- People who want to reduce the appearance of a "thin lip" when smiling (upper lip that disappears on smiling)
Lip filler — best for:
- People who want a real, visible increase in volume
- People who want to define the lip border (vermillion border, Cupid's bow)
- People with asymmetry between upper and lower lip
- People who want a result that lasts longer than 4 months
- People who want to restore volume lost through ageing
Can they be combined?
Yes. Combining a lip flip with lip filler is a common approach when both volume and maximum eversion and projection of the upper lip are desired. The doctor assesses each person's anatomy and determines whether the combination is indicated and what the ideal balance is.
Note: the combination is not always necessary. In many cases, one treatment alone achieves the goal.
Recovery and side effects
A lip flip has minimal recovery. In the first few days there may be some difficulty pronouncing labial sounds (p, b, m) or using a straw. This effect is temporary and resolves as the muscle adapts.
Lip filler causes swelling in the first 24–72 hours — the final result is only visible at two weeks once swelling fully resolves. Small bruises may appear, especially in lips with more vascularity.
Which should you choose?
The answer depends on the goal. If the question is "I want fuller lips" → filler. If the question is "I want my upper lip to show more without looking too full" → lip flip. If both → a combination, with medical assessment.
At Cosmo Clinic, the consultation always begins with an analysis of lip anatomy and patient goals. The available lip treatments are described on the lip flip and lip filler treatment pages.
